A Case of Severe Hemolytic Anemia Associated with Heterograft Replacement of the Mitral Valve









Abstract
Hemolytic anemia as a complication of prosthetic valve replacement has been well documented. Hemolytic anemia usually occurs in aortic valve position with mechanical valve but rare in mitral valve position especially in case of tissue valve. Cardiac valves fabricated from biologic material are associated with a reduced incidence of hemolytic anemia. Hemolysis was recently reported in patients with an Ionescu-Shiley pericardial xenograft prosthesis in the aortic position but not in the mitral site till now. A 41-year-old housewife was admitted to our hospital due to suddenly developed dark colored urine. Echocardiographic examination showed mild degree mitral regurgitation with valve thickening. There was no definite-evidence of para-valvular leakage. The peripheral blood smear showed normochromic normocytic anemia. The hematologic and urinary examination revealed severe hemolytic evidence disappeared completely. We are reporting a case of severe hemolytic anemia associated with heterograft replacement of the mitral valve with a review of the literature.
